Important information about team accounts:

This form is used to set up the lead account holder and up to 10 team member accounts for your organisation.

Please complete the details for the main user (the person completing this purchase) and any team members you already know will require access.

If you are not yet sure who the remaining team members will be, you may leave the additional team member name and email fields blank. These accounts can be added at a later date either:

via your account dashboard once access is live, or

by contacting us at [email protected]Ā 

Once team member accounts are created, each staff member will be able to log in and follow the guidance provided to set up their own personal Wellbeing Monitor account.

Purchase Orders are dealt with manually and Wellbeing MonitorĀ accounts are not available until payment has been received.

For immediate access, payment should be madeĀ using a credit/debit card.
