Submit Purchase Order for 'Organisation Wellbeing Monitor Staff Accounts'
Important information aboutĀ Organisation Accounts:
This form is used to set up the lead account holder.
For an Organisation, you will receive an automated email after submitting this form with an Excel template called: Wellbeing_Monitor_Organisation_Staff_Account_List.xlsx
Please complete the Excel template with your team member details and email it back to us at [email protected]Ā .
If you donā€™t yet have all the team member details, thereā€™s no need to rush. You can complete and submit the template later. The Excel template will also be available to download from your account dashboard once your account is activated.
Once team member accounts are created, each staff member will be able to log in and follow the guidance provided to set up their own personal Wellbeing Monitor account.
Purchase Orders are dealt with manually and Wellbeing Monitor accounts are not available until payment has been received.
